*Referral REWARDS Program Rules & Restrictions:
Reward is based upon 2%+ of the total sales price of your referral’s first purchase ONLY. Award claims are the sole responsibility of the customer. You must notify EXOVATIONS® of your referral using the form above prior to your referral’s first sales appointment. In accordance with IRS guidelines, you must provide EXOVATIONS a completed W-9 form in order to participate in this program and receive cash rewards. Your completed W-9 must be sent to EXOVATIONS no later than 10 days after you submit your online referral. Failure to comply with these rules will result in forfeiture of cash reward. Award checks will be sent 4 to 6 weeks after receipt of your referral’s final payment. This offer is not valid for EXOVATIONS employees and their families. Program may end at any time without notice.
